Status	Meaning
`400`	Malformed JSON-RPC request or missing required parameter
`401`	Missing or invalid `Authorization` header
`403`	Token lacks `mcp:patient-state` scope, or patient not in your organization
`404`	Requested patient or resource not found
`500`	Internal server error; contact support
